Your First Visit

At A-Care Chiropractic & Acupuncture in Richardson Tx (near N. Dallas & Plano), our goal is to develop a treatment program and plan that will put you on the road to recovery.  You can print and complete the New Patient Form before your first visit or we’ll have the form ready for you to complete.

Your first visit will begin with us making a copy of your license and insurance card followed by a consultation and examination with Dr. Leybovich.  The initial exam consists of a series of questions related to your medical and family history, detailed orthopedic, neurological, and palpatory tests, to further evaluate your condition.

Insurance Coverage
